Disease management is an approach to improve the health of patients with chronic conditions by teaching them how to manage their chronic disease(s). Patients will be educated on how to responsibility take care of themselves and avoid potential problems or worsening of their health conditions.
About 48% of professional drivers have a chronic condition (diabetes, hypertension, obesity) that restricts their Commercial Driver's License to 1 year or less. Due to the uncomfortable living conditions, working extremely long days, high-stress environment, remaining sedentary for long periods of time, the lack of quality food options, and the lack of convenient medical care on the road many drivers do not treat their chronic conditions. This not only affects their ability to drive, but it also costs the trucking industry billions of dollars in productivity loss.

Omada offers virtual programs focused on prevention, diabetes, and hypertension. These programs focus on changing your everyday behaviors to help improve your health. Each program is adapted to your needs and can be followed at your own pace.
Roehl is providing this programming through Omada AT NO COST to all employees and dependents on the Roehl medical plan 18 and up that meet clinical requirements.

ConditionCare is a no-cost health and wellness program that provides tools, resources, and support to Roehl employees and their dependents on the Medical plan with these conditions:
- Asthma (pediatric or adult)
- Chronic Obstructive Pulmonary Disease (COPD)

- Diabetes, Types 1 and 2 (pediatric or adult)

Anthem will call you or you can call them toll free at 866-962-1070. They will verify your identity, ask a few questions about your health, and invite you to join the program.
- To get the best experience it is encouraged to register on anthem.com and opt in for email communication.
Program Process
- You will receive educational resources (email newsletters)
- Nurse care managers, dietitians, and other health care professionals will support you in reaching your health goals.
- You may need to complete a health questionnaire to help the health professionals better support you.
- Lastly, a nurse care manager will provide you guidance on reaching your health goals and will be reaching out periodically to offer encouragement and advice.
