Benefit Enrollment
When and how do I enroll?
You must enroll in benefits prior to your effective date of coverage.
- Experienced drivers: First of the month following 30 days of employment.
- Student drivers: First of the month following 60 days of employment.
- Non-Drivers: First of the month following 60 days of employment.
Employees can complete the enrollment process through online enrollment or by speaking with a benefits enroller. If you wish to have your spouse complete the enrollment process for you, please complete the
Employee Authorization Form and return to the Pay and Benefit Services Department.
Once you have enrolled, your benefit counselor will send you a link with instructions on accessing your benefit summary. Please follow the instructions and review this document for accuracy. Contact the Pay and Benefit Services Department with any updates or changes.
Adding Eligible Family Members
During your enrollment, you will need to verify your personal information. This includes your legal name, address, date of birth, marital status and social security number. If you are enrolling eligible family members, please be prepared to provide their legal name, date of birth and social security number at the time of enrollment. Once you complete your enrollment, you will also be required to submit documentation showing their eligibility for coverage. Acceptable documentation includes:
Documentation for Legal Spouse:
- A copy of your government issued marriage license (must carry the seal of the office documenting that the license has been recorded) AND a completed Spousal Coverage Statement.
Documentation for Dependent Child Under Age 26:
- Biological Child: A copy of the government issued Birth Certificate identifying you as the parent.
- *For a newborn child less than 31 days old, the hospital issued birth certificate will be accepted.
- Step-Child: Copy of the government issued Birth Certificate identifying your spouse as a parent AND a copy of the government issued Marriage License showing you are married to the parent.
- Adopted Child: Court document showing adoption placement, petition for adoption, or final adoption certificate.
- Legal Ward/Guardian Child/Foster Child under age 18: Court or agency documentation AND a copy of the government issued Birth Certificate.
Documentation for Adult Disabled Child:
- A copy of the government issued Birth Certificate identifying you as the parent AND medical certification of disability prior to age 26. If the adult disabled child is your step-child, a copy of the government issued Marriage License showing you are married to the parent is also required.
Please note coverage for your spouse and/or children will not go into effect until this information is received.
- If this information is not received within 30 days of your effective date, your dependents will not be added to coverage. Your next opportunity to enroll will be during Open Enrollment or if you experience a qualifying life event.
- If this information is not received prior to your effective date, you may owe the premium difference in employee only coverage and employee plus coverage back to your effective date.

You will receive an email upon review of the documentation.
If you do not enroll as a newly eligible employee, you will not be eligible to enroll until the next open enrollment period or you experience a qualifying event. All qualifying event questions should be directed to the Pay and Benefit Services Department.

Online Enrollment.

- Username: Employee code
- Password: Last four digits of your social security number

Option 2
Over the phone by calling the enrollment center.
Call the enrollment center at 1-833-781-5331 Monday-Friday from 8 AM - 5 PM CST.
If all benefit counselors are busy, you will have the opportunity to leave a message. Please follow the prompts. Calls are returned within one business day.
You will be guided step-by-step through the benefit selections for which you are eligible, the level of coverage and the cost (if any) to you.
Over the phone by scheduling an appointment with the enrollment center.

During your appointment time a benefit counselor will call you to explain your benefit options, answer your questions, and assist you with the enrollment process.
