Preventive Care



Maintaining a healthy lifestyle is a goal that we should all set for ourselves.

We encourage all of our employees and their family members to make healthy choices, beginning with an annual wellness exam.
Regular preventive care visits with your primary care provider are vital to your health.  These visits help identify health related issues early, as well as assist in managing any chronic health conditions.  Research shows that individuals with a primary care provider spend less time in the hospital and less money on healthcare costs throughout their lives. 

There are no benefit limits on the Silver or Gold Plans for preventive care. This will allow you to obtain your wellness screenings using in-network providers at no cost to you.

Medical Premium Discount

Employees on the Roehl medical plan and their enrolled spouses who complete a visit with their primary care provider can qualify for discounted medical premiums.  This discount can save you up to $286 per year for Employee Only or Employee + Children Coverage or up to $572 per year for Employee + Spouse or Family coverage (please note, to obtain the full discount both you and your covered spouse will have to complete a primary care visit). 

To receive the discount for 2023, you must provide documentation via this form showing a preventive care visit within the past 12 months.  

To receive the discount for 2024:
  • If you complete your visit between January 1 - August 1, 2023, no action is required on your part.  Roehl will be notified of those who qualify to receive the discount prior to Open Enrollment, meaning you will be able to see the discount as you complete the 2024 Open Enrollment process this Fall.
  • If you complete the visit between August 1 - December 31, 2023, submit this form to Human Resources.  

Don’t have a primary care provider?  Simply log into your account at www.anthem.com and select “Find Care & Cost” to find a provider near you.
